Did you remove the mag disconnect plunger and spring from under the rear sight? Or did you remove the lever? If you removed the lever, that is the ejector, the gun will not eject an extracted spent casing without it.....if the cases are getting crushed between the slide and barrel, then the extractor is not the issue, when an extractor fails to do its job, the spent case is left in the chamber, sounds to me like you either have an ejector issue, or you removed it, to disable the mag safety you only need to remove the spring and plunger, which requires removing the rear sight, the ejector also acts as the mag safety lever, but must remain in place for the pistol to function.....
