Wanted to post back on this. After a few more trips to the range the hold open empty issue occurred more frequently. And on the last trip the slide started sticking.
Sometimes it stick going backwards and other times going forwards. The guys at the range thought I assembled it wrong so they cleaned it, lubed the hell out of it, took some shots and it jammed for them as well.
In the pic of the slide you can see the gouge forming, this is where it is getting stuck. Second picture shows it stuck.
No problem with S&W, they are sending out a FedEx label. Will post back when I get it back.
