I sent in my form on or about 1 Dec 2016, for my K-22 history. As of today I have not received a reply. They did cash the check. Today I sent an e-mail to the address given on the historical foundation web site asking if I could get a status report on my request. I came back undeliverable.

I may have sent the form to the wrong address. The address on the form is in Warren Ma, the address given here and other places is in Kansas.

Any info would be appreciated

If you can give me a correct e-mail address I could send them a duplicate.
