As to what happened, I have no idea. Maybe there was a void in the wire that the swaged bullets were made from. I think they get this lead in a wire form and cut it to make a swaged bullet, but I'm not sure.

I too came to the conclusion that there must have been a void in the lead wire bit with which the bullet was punched.
The fact that it wasn't a hollow base is quite intriguing though!
