What holster were you using? Was it made to fit your Model 19?
I've been carrying K-Frames IWB for years, nothing like that has ever happened.

The only thing I can imagine is that your holster didn't fit your revolver very well, and then it collapsed inward on itself (possibly from a lack of reinforcement), squeezing the revolver between your body and your belt and pushing it upward. With a quality holster this seems extremely unlikely.
