Actually, this has happened to me.

I blame it on an oily, empty round in the chamber. I put one in there each time in order to dry fire it safely before medium-term or long-term storage -- it acts as a snap-cap.

The oil acts hydraulically against the walls of the chamber, making the round "stick" in the chamber. It can be VERY hard to move the slide.

This has actually happened twice.
