Default Off my hip, under my arm......

Due to very major surgery to my Aorta and other related areas, I cannot wear my conventional OWB from DeSantis. They zipped me open from breastbone to groin.
After research on this and other forums, and several talks with LGS and range folk, I purchased an "executive" shoulder holster from Galco. Some say it is pricy. As we learn 'the price is not always the real cost of things'
The materials and construction are second to none. A couple minor fitting adjustments and it feels as natural as though I have worn it for years.
Any one of my 3 J-frames snuggle right in and ride nicely walking, bending or just driving around.

I'm not in the 'quickdraw' mode when I carry any of my guns so at least for now things are the way I like'em.

I am 6'-1" and 175lbs. medium build and this rig is very comfortable for long wear times.

what about the undershirt with velcro pocket from 5.11 tactical? I keep looking at those but not sure how well it would work and how comfortable it would be. I suspect it would work better for a relatively thin semi auto than a revolver.

Back a couple of years ago, I was flying around in helicopters quite a bit for work. I got a Miami Classic and then a VHS holster for this detail. I was very happy with the fit and finish quality of the Galco shoulder system. I used the horizontal (Miami) for carrying S&W M&P pistols. Flying around in the left seat with the doors off made me feel a little worried about my pistol falling out of the horizontal holster, but it never happened. I got a vertical (VHS) holster for the Sig 220/226 and used them both for a couple more years. Both had a tight hold on the pistols they were designed for, and the harness felt comfortable while sitting in the helicopter or scrambling around the desert working a rescue or other job that needed doing. I agree with you on the cost aspect of these. Quality built holsters last a lifetime and these are high quality in every way.....

Glad you're doing ok. I had a similar experience lately (different kind of surgery which prevented my normal belt carry) and felt the need for an alternative system. Luckily my recovery was quick, so I never made the plunge, but I've been reading up on shoulder holsters and am glad to hear the Galco Executive works as described. Best wishes in your recovery.

Glad to hear that you are doing ok and are still able to carry with comfort. From what I been hearing, shoulder holster carry seems to be making a comeback. Be safe.
