Carrying it cocked and locked is going to be the issue. Holsters for TDA/DAO pistols with a thumb break retention strap usually will not fit properly between the slide and cocked hammer. If you are going to carry cocked and locked, then an open top holster is likely the best bet.
Finding holsters specific to the 745 and even the 645 and 4506 can be difficult these days. I have found that holsters for the Beretta 92 and the Sig P220 often fit the big S&W just fine. The Beretta 92 holster thumb break straps will usually close almost perfectly over a 645 with the hammer down. The thumb break on a P220 holster is actually a little to big, allowing the 645 with hammer down to move a little.

I found a used Bianchi 19L on eBay that is made for a 645. Also have a “Cuda” from Simply Rugged that works well. I’m sure Rob can make you one. Delivery time was pretty quick when I ordered mine.

S&W #645..#745 holster
I have had wonderful luck with the older Bianchi Accumold holsters. #645 fits like a glove..I'm pretty sure a #745 will fit as well. Wrap the pistol in wax paper and push it into the holster, tight fit. leave the wax paper wrapped pistol in the holster for a day or longer if you can. Holster the pistol in and out several times. The wax from the paper will coat the inside of the holster and allow the pistol to slide in and out with ease. Repeat until your happy. Bianchi Accumold holster #15, mine is an adjustable paddle ..Works perfectly for cross draw. Adjustable cant all directions. I have 3.... check it out..

Maybe a simple belt slide, like mine shown here, would get you by until
you find something better, or have something made.
