Default Not a Smith, but a revolver question

OK, I need some help here, customer brings in an old Colt Official Police. overall gun is in good shape (80% or so) Everything works, nice lock up, hand & star no appreciable wear, trigger & hammer likewise.
...works fine 100% in single action mode. Double action will rarely fire, hits the primer at almost perfect 12 o'clock hit on top of the primer. Recoil plate is not loose, no real wear or scuff makes that I can see on the inside of the frame/back of the recoil plate where the hammer/firing pin goes thru the frame.
We just installed a new mainspring (v-spring) same deal.
Any suggestions would be greatly appreciated...this one's about to make me crazy!!

Phil;
The OP/OM is my other favorite .38, so kind of shooting from the hip here: Short firing pin ? Short DA sear ? Hammer dragging on frame ? Rebound lever dragging on hammer ?

Try another mainspring.
Is the cartridges fully seated??

Make sure you put in the right spring, as the official police was manufactured in different calibers. .22 and .38 off the top of my head and more then likely the springs are different strengths for the different calibers, just like all other firearms for the most part.
