It is a sure enough 3rd Model Single Shot (also known as the Perfected Model---after the revolver frame/DA action it shares).
The 6" barrel (if original) is right up there with hens' teeth when it comes to rarity--although the 8" version seems to be even more rare (which is to say I have seen 6" versions now and again---I have a 6" version---and I have NEVER seen a legitimate 8" version). If the barrel address is centered, one may righteously presume it was born that way---never mind it at least appears to be missing its front sight. The "Thin Round Top Blade" (with a U notch rear) is likely the correct sight (if for no other reason than that's what's on mine)---probably can be found with not too much difficulty---can also be made rather easily.
My 6" (#4807) was shipped November 3, 1915. My next one (#7642), a 10", was shipped February 17, 1916. Yours likely (but not necessarily) went out in between.
The value of yours is considerably less than would be the case given better condition (finish)----say $5-600 as a "shooter"---perhaps more if the buyer just absolutely HAD to have a 6" version---and was bone weary from looking for a better one.

I should have commented on "matching numbers"----the latch/rear sight assembly should be numbered to the frame---as should the barrel. The number (if any) on the latch will be found on the bottom side. The barrel number will be found on the rear of the barrel, inside the recess where the latch/sight lives---on the right hand side. Mismatched numbers (or no numbers) detract significantly from value (but not so much for "shooters"). As an aside, the ejector also is/should be numbered (back side)---as should the grips (inside right hand panel). The grip number will be written in pencil, and will range from difficult to impossible to see. VERY good lighting and varying viewing angles help.
