Serial #564911 is an M&P model which probably left the factory about 1927.

Welcome to the Forum.

M&P is shorthand for the Military & Police revolver. It is a K frame, kinda the middle of the sizes available from S&W. When it was built, Smith & Wesson had names for all of their firearms. In 1957, the factory assigned model numbers, which were used along with the names.
