Most likely dirt or debris in the hand slot, and/or excessive end shake on the cylinder or yoke. Dirt or debris under or on the extractor can also be a problem. Have you cleaned it thoroughly?

What ammo are you shooting? Reloads, factory, +p ? I'm asking cause a bullet could be backing out of a case and hanging up the cylinder.

Are you shooting DA or SA? Did you reset the trigger by releasing fully between shots ?
Try this; with the gun UNLOADED, point the muzzle down and pull the trigger five times. Did the cyl. fail to rotate ? Now point the muzzle up and do the same. If it rotates with the muzzle down, but not up, chances are you have a broken hand spring. If rotation is inconsistent, it may just need a thorough cleaning with the sideplate off.
Please let us know what you find.

Any time you load and close the cyl of a S&W revolver it's a good idea
to always rotate the cyl until the locking bolt drops into one of the
slots on the cyl.
