I ran across this image the other day on the internet
and had never seen it before. I found it pretty interesting.
Does anyone know exactly what gun this is? Thanks for
your insight.

Default tunnel rat

Not sure what all the equipment is. He appears to be a tunnel rat .During th e Viet Nam war those guys went on search and destroy missions in the Congs tunnels.

I am aware of QSPR's, but this isn't the model 29 that I am used to seeing. And the quietness of the tunnel gun is in the ammunition, not a silencer on the end of the barrel.

From what I read years back the "rats" guns were modified to have extremely small gaps between the cylinder and cone. So tight that they could only get off a few shots without a good cleaning. If that is true a "silencer" would most definitely help!

More info:
IIRC, in 1966 the Limited War Laboratory developed 6 suppressed prototype "tunnel rat" guns based on an S&W Model 10, with almost no cylinder gap. They sent the 6 to Vietnam for testing followed by an additional 450 units. By the time the 450 units hit the field the LWL decided a suppressed .22 was a better choice, so not many of these revolvers were ever made.

IIRC the QSPRs came along pretty late in the game and not that many were produced or used. Tunnel Rats used a variety of guns during the war, including the "silenced" Model 10 shown here. The only "Rat" I met during my time over there was using a Model 10.
