If you open the cylinder and look just above it's hinge point you will find the model number. As clang444 said, it is a 686.
Also the number that you posted is not the revolvers serial number, that is the WVSP property number. The serial number will either be below the model number or in the butt of the firearm. The Pachmayr grippers that are mounted on the revolver hide that second possible serial number location.
As to value for insurance purposes, that depends on if you have a current value policy or a replacement value policy. You would probably have to spend $650 plus sales tax to buy a new one. A current value policy would probably give you $300 for the loss.
To determine age, we would have to know the at least part of the real serial number. Mine was manufactured in 1987.
