Default what is happening

My wife has a model 67 4". I can shoot it all day, as fast or slow as I like with no problem. But when she shoots it , double action... when she goes to unload it, sometimes there will be an unfired shell left. happened twice today ( she shot 200 rnds through it)
I watch her, she shoots it till it doesnt fire anymore. The unfired cartridge doesnt have a primer hit at all. Only DA, only when she attempts to fire fast. its like it skips one..???

This is caused by not letting the trigger fully reset between two trigger pulls. First double action pull fires a round, then the trigger is only let forward about half way. From this position, the trigger is pulled again and the cylinder rotates to the next chamber, but the hammer is not fully cocked to fire the round. Trigger is released and reset, and then fired again DA. Three trigger pulls, two round fired.

I figured it was something she was doing... just wasnt sure what...................

Some dry fire practice will help correct this. Watch as she does it, keeping your eye on the hammer not the cylinder. The hammer needs to go back fully and release each time.

I watch my students like a hawk when they are new to revolvers -- a lot of them start pulling the trigger and then let off and start pulling again -- then they wonder why they skipped a round.

I teach them to pull the trigger firmly but slowly (in the time it takes to say "Momma Mia" -- I tried to come up with the four slowest syllables in the English language and still had to go to Italian).
