4" 681 Performance Center
Did the PC ever produce a 4" 681?

Yes - see below highlighted and underlined portion of Model 681-5.
The two below 681 models are Performance Center models:
Model 681-4 Lew Horton Special, 3" ported barrel, 7-shot, chamfered cylinder. About 300 produced. Serial prefix LHB.
Model 681-5 Special for Camfour, 7-shot, chamfered cylinder, (drift) adjustable front sight, ported barrel, matte (glass bead) finish, product code 170172 for the 4" barrel and product code 170178 for the 3". Circa 2001.

Stupid question, if the 681-5 has an adjustable rear sight why wouldn't it just be numbered as a 686?

The book is actually wrong, it came wth fixed rear sight fyi.
