686 frame, 681 overstrike
Re-purposed frame? The frame number must be stamped before the adjustable sight cuts are made. (This is on a fixed-sight gun.)

I suppose it could be that it was originally destined to be a 686, then S&W needed an extra frame or two for a Model 681 order, so before the sight cuts were made, re-stamp it and cut it for fixed rear sight.

I just find it interesting that they stamped what it was "going to be" before any irrevocable machining steps took it on that particular path.

Or maybe the 686 was a mis-stamp and the over-stamp was a correction.

I’ve seen the mis-stamped 681 as a 68-1, this is a new one !
