Default Chipped extractor, 3906

My 3906, with approx 2000 rounds, sustained a chipped extractor resulting in complete FTE. I was shooting Blazer Brass 9mm at the time. Sent it off to SW with an expected lengthy turn around time, but would like opinions as to what is the cause and is this occurance unusual?

Probably just a bad part.
Had a firing pin go bad on a fairly new Springfield 1911 only after 400rds.
If there's any other issues im sure S&W will get it corrected.

Could have been a flawed extractor. One thing that will lead to a short extractor life is dropping the slide on a round already in the chamber. Doing this places an impact force on the extractor hook and can result in damage to the hook.
