Found a two-piece blue box for S&W handcuffs....
Looks just like a gun box, except smaller. I thought it was kinda neat...never saw one before.

Is the box end label:
1. printed with no model number
2. printed with model number
3. printed paper label pasted on upside down
4. newer modern box printed with model number

End looks most like your #2 photo, but the top looks most like your #3 photo. It says Bangor Punta on the top. Instructions are printed on inside of lid.

G-Mac,
The model 90 was introduced in 1952 and I believe they were produced until the late 1970's. Maybe someone with more knowledge can give you a closer date on the box. I believe your box looks like this. (bad lighting, box is dark blue)

Thanks for your efforts...that's exactly like the box I have. It was with a bunch of handgun boxes that I bought from a retired police officer.
