I also had an OOB. S&W repaired, Nooo problems
I purchased my second M&P 15-22 on 22 Dec 2012. Was shooting Winchester 36 grain when the OOB ocurred which blew the extractor out. Sent the rifle back to S&W and they fixed it without question.
FYI. Photos below of the cartridge and the bolt (extractorless).

Mike,
Thanks for the reminder, I have seen this ammo correlate to several other OOB detonations... this is going to be one ammo I am going to definitely stay away from using.!!!!

What's really interesting about this whole OOB issue are the discussion points that the occurances are common (although not frequent) - to .22 Semi's,
- ensuring bolt and chamber faces and chamber are clean
- to the FCG-Bolt closure/action of the M&P 15-22
- related to poor quality/dirty ammo
- a combination of all the above
I am a bit preplexed when I look at all the possible factors. Still, I am very hesitant to use/finish the same brick of Winchester ammo. That brick will more than likely be used up with my pistol, or I'll only do very controlled, slow fire with the 15-22.
Smith and Wesson did me good however. They got me all fixed up. Kudos to them!

M&P 15-22 goes boom...not good
I got my 15-22 yesterday evening, no chance to shoot yet, but browsing through the manual saw a list of ammo they do not recommend...don't specifically remember what was on it, and it's not in front of me but I'm wondering if some of the ammo mentioned in this thread is on that list?..

FWIW, that list dates from the introduction of the 15-22 and may well be outdated and no longer reflective of today's ammo quality. The changes over the past few months in Remington's products is proof of that.
IMHO, today that list, especially the "not recommended" list is useless. Try any ammo and see how it performs in YOUR rifle.

This is a copy from the manual. But I agree, try and see what works. I was shooting Winchester HV 36g, which isn't on the "not recommended" list. But Winchester Wildcats are.
You make a good point. I'm a lot more cautious though, and mindful.
