New 15-22 FTE and fires when safety is on...
Well my new 15-22 has issues. I have had it for about 5 days now and as of today it shoots with the safety on. I have shot about 300 rounds through it, good ammo and I would say that 15% were FTE. I was going to try and clean and adjust it tonight but went out to shoot some new ammo and I found out with the safety on that it fires!. Is this an adjustment that I can fix or do I need to send it out? Thanks.

You have an unsafe firearm.
Immediately send it back or take it to a gunsmith. DO NOT ATTEMPT REPAIRS ON YOUR OWN.
It will cost you NOTHING to return it except a little bit of time away. Call S&W and get a Return Merchandise Authorization (RMA) immediately.
It's not worth it.

I called S&W and got an RMA. I have plenty of guns but I had a whole weekend planned this coming weekend. This is a big inconvenience. Still love the gun though.

Wow-Now that is a first!
There seems to be something amiss at S&W quality and control, but sending out a firearm that fires when on safe, wow. Lawsuit?
This really surprises me, have you removed the grip or the safety yourself? or was this serious defect there without any interference by yourself....because if it was then that is actually really scary. Kind of like sending out a car with no brakes. What is going on every new 15-22 sport seems to have the same FTE issues? simply not good enough.
To this day i have never had one FTE ever, in thousands of rounds.
I would personally be requesting a few extra mags, firing pins, ejectors, extractors and maybe even a S&W range bag to keep me happy!

Before sending it back you might look to see if the trigger or hammer pin has walked. They're short enough they can walk and dis-engage from the lower and still not obviously protrude on the other side.

Sorry for the delay folks. I ended up shipping my 15-22 out on may 18th and just got it back yesterday. There were 2 pieces of paper inside with what seemed like a list of every part on the gun. They didn't tell me what they did or anything. Next to every single part listed it said Repaired then next column it said Replaced. The safety now works like its suppose to and I was able to shoot just one mag yesterday 25 rounds and every one of them ejected flawlessly. I would take a picture of the list but I am at work now. Is this normal for them to send a gun back and not tell you what they did to it?

Pretty much.
Would knowing what they fixed change the end result much? Other than knowledge....

Probably peace of mind. It'd be very nice to know what repairs your car dealer did, right? It's nice to know you'll be fine on a long trip.
Picking my 15-22 up late next week. I'll try and use the range at the shop to run a few mini mags through and make sure she's working okay. I highly doubt there will be anything gunked up/etc. Speaking of gunk, do you guns clean and oil your guns before using the first time or is it shipped good to use? While I'll take a peak at the parts, I don't want to do a full clean if I don't have to while breaking it in.
Congratulations on fatherhood detroit85!

My 9 year old just had a out of battery fire in her new M&P 15 22 sport not even 3 months old. I've contacted S&W already and got a return slip but I'm kind of concerned about having it repaired and handing it back to her afterwards just for the fact it's mostly polymer. the gun has been a complete nightmare with fail to ejects and feeds and now this. been running the good ammo too cci mini mags and ar tacticals, and afew federal automatch

Even more reason I would think to know what repairs have been made
That's pretty scary. Good thing you caught it.

I'm really concerned about the strength of the Upper and lower being made of polymer , honestly I don't know if I want her to shot it if its just repaired. Already been nothing but trouble what if there is some kind of weakening in the plastic

Detroit85,
Congratulations on the new baby and thanks for posting the end results of the repair even though S&W did not disclose exactly what they did.

is this a possible cause of the issue?
From Brett248vista:
Basically, on any AR, the hammer has to be cocked for the safety to engage. If the hammer is not cocked and you force it into safety you can damage the rifle.
So essentially the AR is always cocked and ready to fire (even when on safe, the hammer is cocked) and it should always be treated as such..

I have heard about an empty case getting into the guts and causing this same problem.
