M&P 9 v. M&P 9 Pro Core 5?
Is there any major difference between these two? Which do you prefer?

Other than a barrel that is 3/4" longer, a performance center sear, a slide that is milled for optic installation, and taller sights for co-witnessing with the aforementioned optics, they are exactly the same. (oh I forgot that they have different lettering on the slide too)

But, do these make a substantial difference in the gun, or is not worth the extra money?

What our friend is telling you is that it is a very different gun. An experienced shooter will appreciate every bit of the custom work done by the custom shop from the first shot to the very last shot of the day. It comes with generally every thing that most would like straight from the manufacturer.

That depends on you.
Some will say the extra sight radius helps them. If carrying concealed a 5" double stack gun is arguably harder to conceal (esp. with the taller sights). Some will want to put a red dot on their gun...some won't. A sear is relatively easy to put in afterwards (Apex) if you go with the 'regular' version.
'Worth' is an individual thing and depends on what you're intending to use the gun for.
Rather than comparing a 4.25" to a 5" barrel, maybe consider the 4.25" Core? IMO that's a closer comparison and harder choice but still one where you have to decide what you want it for.

If you don't plan on using an optic, then it's a waste of money(get the non Core Pro), however if you do plan on using an optic, then it's the best money you will spend (you would be hard pressed to get the slide machined and sights moved for the extra money that Smith charges). Of course it's a slippery slope that once you start modifying, you can't stop.

Trigger, sight radius, texture back straps, able to mount optics. Those are the main difference and don't mean to be harsh but if don't know what those differences are then stick to the M&P 9 and learn.

Yes, IMHO it is worth the extra. The tuned trigger alone for a lot of people is worth the extra.

Ronnie What are you going to use it for . Fun gun, house gun or CC also. If your going to also CC the 4.25in is easy to carry and then you still have the option of the core or not. I carry a core 4.25 with optic right now while hand is healing after surgery. Works very well with a far better trigger.
