Default FS45 RSA issues?

A friend of mine had an issue with his FS45. When removing the slide, the take down lever would not stay flipped down. It would flip right back up. If you did manage to keep it down and remove the slide, it would go back horizontal just as you reinstalled the slide.
With the slide locked back, we noticed that the rod could wiggle around a bit. Occasionally looking like it was crooked.
It's seems like the RSA is popping out of the small grove on barrel. The RSA does not appear bent or damaged.
Has anyone else experienced this before?

The gun shot great. No other issues.

Maybe the groove on the barrel was not machined properly.....maybe at an angle allowing the end of the rod to slip off?

What you're experiencing is not really an issue. Just hold the lever down when removing the slide. It's popping back up when the slide is installed is a good thing because that's the intended finishing position anyway.

The link posted above includes a method to modify the take down lever a little so it won't do this anymore. You could do that, but in my opinion it's unnecessary.
