A couple of questions come to mind.
It was a 2008 rifle. Was S&W building all their own rifles then, or was it still Stagg Arms at that point?
I wonder if it somehow fired out of battery?

I'd be willing to bet it's gonna be an ammo issue.

+1, absolutely. Not the first and not the last. No design or brand is immune from out of spec or defective ammo OR operator error.
Obstruction in the bbl. is most likely cause.

I have a little contact at S&W and called her to ask about what happened. Here is her email back to me.................."Hey Pete…thought I’d let you know I contacted Paul Pluff, and he confirmed that once all the testing is complete that a statement would be released….thought you’d be glad to know that (as I was)…take care, ".... I left her name out of it. Anyhow Paul Pluff is a big wheel at S&W , I forget hsi exact title.
Anyway I'm sure they will let us know in time. Pete
