Put a small dab of silicone caulk on the top of the magazine follower, where it catches the slide stop. It's not pushing it up and enough to catch the slide well. May have to reapply every so often.

I didn't see any mention as to whether this was purchased new or used. If you ruled out lube, ammo, mags and recoil spring then it may be finger placement.

I went through this with my Sig and aftermarket grips. My hands are large and the aftermarket grips where very thin (which is why I purchased them). Turns out my thumb kept interfering with the slide release causing the slide to close on the last round. I remedied this by reinstalling different grips. Unfortunately you can't do that with the sd9, unless you try installing rubber "slip on" grips (if available). I'd pay particular attention to finger placement next time at the range to see if that's the issue.

I feel you I have as of now have shipped mine back about almost 2 weeks now to S&W for the 2nd time. The first time had the same lock back issues, stove pipes FTF, FTE put it this way the kitchen sink of issues.
This time, would put 6-8 rounds out of each mag, all 4 that is on target hitting center mass. The rest of the mag with nothing changed in stance or POA, all the other rounds hitting low, high put it this way everywhere.
I had on both occasions have several other people at my local range shoot it with the same results. Well at least now I don't feel like I'm the only one that got a lemon anymore.
I am hoping that this time it's fixed or it's getting sold or used for AK target practice.
