My GUESS is that: That round was fired in a chamber designed for a larger caliber (ie...9mm in a 40 cal or something like that)

lol you got it. took the new matching glock 26 and 27 's to the range today (finally) and i was in another lane shooting my rifle. my loving wife asks me why my "****** glock 9 kept jamming" i pulled this out of the chamber and showed her on the side where it said 40 in big print...

LOL. I remember reading a story somewhere online where a copy was out on the range qualifying with his Glock 40 cal. His buddies were scoffing and deriding him for his choice of firearms. Meanwhile, he was getting really frustrated because he was not shooting nearly as well as he knew he could.
Turns out- yes, you guessed it!- they were providing him with 9 mm ammo, and it was shooting in his gun.
