Default Malfunctioning 669

Hey Guys,
A buddy of mine asked for help with a wonky 669, which was 100% reliable, but after some " safe time " has several "failures to eject" per mag. The fired round stays in the chamber and requires a stout yank on the slide to clear the fired cartridge. I assumed just a dirty chamber, or maybe a extractor issue.... But after field stripping.... I noticed this on the under side of the barrel lug....

To me it looks like a defect in the barrel forging... But I am no expert.

What do you guys think.... Time to call S&W? Would you think this would be a warranty repair? The gun is obviously over 25 years old... But is other wise in nice shape... And is one of my buddies favorites!

P.s. I am a L.E. firearms instructor/armorer and this is an off duty piece.

It's hard to tell in the pics, but it almost looks like a piece of something has fused itself to the barrel. What does the slide stop pin look like?
