Have to agree with SVT28 the right side lever is ugly and unnecessary unless your a southpaw.
The 645 is a neat gun, had one on my short list years ago to go with my 745 until i sold off all my .45's.

I was going to recommend Jack First, but it looks like they are down for a re-opening. I bought a few items from them in the past to fix my 4566.

Well, a 4506 ambi will work fine. Your going to have to piece it
together as there is not one source for all, or a complete 1-5
assembly. (Combination of Midway and Numrich).
Many of my second gen's were missing the right lever and
screw, so I got the price down if I could, and put 3rd gen
safeties on. Most of my second gen's I just changed them
out anyway. I kept all original parts, and am always looking
for levers and screws.

My buddy bought a 645 at an LGS, they told him there was a problem with the screw on the right side staying tight. He used loctite and that didn't keep it secure. Next he used JB Weld. I bought it from him a few weeks ago and I'm looking forward to getting it to the range to see if that works.

Send a PM to Nyteman 44 here on the forum. He has quite the secection of parts for 3rd Gen 45's. I'd be willing to bet that he might be able to help you with your 645. He was very helpful to me and is a fine gentleman to boot.

Had a 645 for a duty weapon once. I couldn't keep that screw tight no matter what I used, Consquently, when ever I shot the weapon the decocker would come loose and swing around. Very distracting. Eventually I just took it off and kept it that way. Looks like the previous owner did the same and filled the thread with a hex screw.

That's a common problem, as the right side screw would fall out, & many lost the lever. I recently purchased a 645, also for $400, & the previous owner replaced both safety levers with a 4506 set up. Those can not fall out. I don't care if it's not "factory original". I do know it's not going to fall off. GARY

The grips came on the gun. I think they came from S&W school. They don't have medalions.

I have one of the 100 or so late model 645s built up with4506 parts. It has 4506 ambit safety and other 4506 upgrades on a 645 platform! It is a big honking hunk of steel that will eat whatever you feed it including empty castles!

I saw a Model 745 with similar grips the other day but am unable to remember if they had medallions.

Great pistol. I lament the passing of the third gen guns and simply do not like the MP line-up. I remember when the third gens came out, they generated a great deal of excitement. They were a marked improvement over second gen guns in terms of ergonomics and trigger pull.
I carried a 4506 as an authorized duty weapon and its heft was reassuring. Since these guns are no longer being supported by SW, I'd grab spare parts to keep the 645 running for a long time. Springs, maybe an extractor, etc.

There's a set on ebay right now for "buy it now" price of $57... search for "s&w 645 grips" in Sporting Goods category.
