I am willing to bet that it varied a bit depending on model, year shipped, maybe also depending on if it was a large order (for say an LE organization or perhaps a large distributor such as Lew Horton or Zander's or RSR, etc)
Best as I can recall, I have only gotten a single 3rd Gen new (sort of) so what I report is only one example.
In a 1006 that was purchased in early summer 1991, it was the blue one-piece flip top cardboard box with the white inner cardboard pistol-shaped box with the little black "top hat" that goes through the trigger guard, it had the pistol, two magazines, a cleaning rod, a wooly mop, a bronze brush, , a very general owner's manual that basically covered all S&W semi-auto pistols (but oddly enough... a 1006 was pictured!), a warranty card and the brown anti-rust paper that S&W always uses.
I *think* that was everything.

My 442 some years ago came in nearly identical shape with fewer items (obviously no magazines) and the generic J-frame manual. My M&P40 was used but came in a blue plastic clamshell case, shell casing still with it, rod, two magazines, three backstraps, manual, and lock.

So basically the same things that S&W has been putting in boxes forever. . I wonder if there was a tool to adjust the rear sight on the adjustable sight models? Chamber flag maybe?

Well "basically" sure. I guess.
With other S&W I have seen additional tools (wrench for 52 or X-frames including swappable muzzle brake) and I have seen pamphlets for genuine Smith & Wesson holsters, separate leaflets for old 70's/80's S&W branded ammo (Fiocchi made, if I recall?) and other different pamphlets, sometimes detailing the warranty and different contact addresses for authorized warranty work, sight adjustment tools, maybe some extra fiber-optic piping for a front sight, moon clips for the few models that used them, lord knows what else over the years depending on model.

Thanks for the picture. I now know what the sight adjustment tool looks like. Now I can try and find this stuff. Nice looking pistol btw.

Yes. I've had a couple that had the orange rubber empty chamber flag with them.
