Default Model 669

669 missing right side safety lever. When I shoot the gun, round loads into chamber, but will not fire unless I lower left side safety into lock and then back to off. I have to repeat this for every round. I bought this gun 30 years ago and just went to range for first time since I bought it (has benn in closet 30 years and gun is in mint condition) and that is when I realized gun had this issue. Does the removed lever need to be replaced in order to shoot properly? I am unable to find part online. Thanks.

I am relatively new to the world of S&W autos, but in my limited experience, the right side lever and its retainer (either screw or pin and spring) are just along for the ride... the body of the safety, with its several grooves, notches, and holes, does all the work. If the main body of the safety is right, the right side lever should be totally extraneous.

I went through the same loss you have experienced on my Model 39-2 and it continued to function. There are several vendors who still have these old parts but you need to really dig, that’s for sure. I ended up buying a whole Model 439 slide from one of the Gun Broker dealers and harvesting parts including the whole ambi safety assembly.

Back to your specific problem, I’m going to guess the problem may well include your safety/decocker, but it’s probably not the right side lever... it will probably require at least a field strip and checking what is going on with all those little levers coming up from the top rear of the receiver.

Numrich has the right side safety lever for a 469 it’s blue but functions the same. 669 stainless is out of stock.

You can contact S&W to see if they will service it for you. Requiring a safety reset to fire the weapon is a demonstrable malfunction of the action and needs to be looked at if giving the pistol a detail cleaning doesn't correct it.

The 669 was subject to a safety-related recall.

It's always possible your issue could in some way be related to this (assuming your pistol falls in the serial number range of the recall and fails the test).

So it will only fire in double action mode only? Absolutely will not fire in single action? That is definitely not right and I do not think the safety has any role in this malfunction. Sounds to me like this problem needs to be addressed by S&W or a qualified gunsmith.
