They don't do a thing to the cylinder. The hammer nose on the hammer is replaced with a slightly shorter one that is also smaller in diameter, and the recoil bushing (the part with the hole that the hammer nose goes through) on the frame is replaced with one that has a smaller hole to help prevent primer flow back that locked up an occasional gun or two.
