You use that hand to shift. That's another difference between Europe and the USA: Most cars over there have standard transmissions, even cars that are sold here as automatic-only.
This past May, I spend 10 days in Germany and Austria. My rental car was a Ford Kuga (sold here as the Escape)...with a 6-speed transmission.
