Pierced primer will cause that. Have a couple like that. Not necessarily a pressure load as some primers are very soft. High pressure through a small hole in the primer eats away at firing pin. Mine still shoot fine. If real bad it might affect ignition but would have to be extensive. I had a batch of Remington small pistol primers that were bad about it even with wadcutter loads
