One place I haven't seen mentioned is
Privateer Leather. I've got three of his holsters - IWB and OWB for my 2" Detective Special 3rd Issue and an OWB for my 3" M10-7. The quality is excellent and his wait times (currently 13-14 weeks) are accurate.
His ordering process is different. You order via email, he replies with questions to make sure you're both on the same page, and you have the option of paying all of the price or just half up front with the remainder due when the holster is done.
