Typical Chevrolet cab. Cab mounts have rusted thru and thus the tilted cab. He can probably spit his tobacco juice thru the holes in the floor.

Cab and Bed Alignment look like bent frame.
That can be straightened.
Suspect owner is happy with truck as it is.
I have driven a Dodge Dakota with a more noticeable "dropping center" bend.
No problems driving - just looked ODD!
Wheel alignment front - rear was straight.

I’m sorry that I cannot post a picture here but here’s my story like the OP showed in the picture.
A couple of years ago I thought that I hadn’t done anything just for fun in who knows how long.
So I went to the Halloween store and bought a plastic life size posable skeleton. I put it in the passenger seat of my truck and there he sat.
EVERYONE LOVES IT!
They said it was hilarious. I immediately received the goal of making others smile.
After a couple of weeks I took it out. The lady at the coffee shop asked why and I said the novelty wore off. She said what about Christmas?
Then it clicked in my mind!!! So a couple of weeks before a special day he gets dressed up and put in the truck.
It makes people smile, give thumbs up, pictures at stop lights, etc. my coworkers love it.
And keep in mind I’m not the type of person who is into ghoulish things.
I was listening to my phone and a Grateful Dead song popped up. So his name is Garcia.
Thanksgiving, Native American headband with construction paper feathers holding a small pumpkin.
Christmas, Santa hat.
New year, party hat and a blower in the mouth.
St pats day, green chapeau with gold buckle and an orange beard and a cob pipe.
Just a few examples.
Normally there are accompanied by sunglasses, corn cob pipe or cigar.

@meaneyedcatz; my dad's ashes were on mom's mantel. Every holiday she would put the appropriate hat on his urn. Mom's ashes now sit next to dad's on my sister's mantel.
The hijinks continue.
