Sounds like after reset the trigger is not cocked automatically, but then you slightly rack the slide, the trigger gets manually cocked, and then you can proceed to fire the round? Maybe worn parts like the Sear spring? Is your sear plunger 1/8" or 1/16"? Trigger bar? Have you done the Trigger...

The replacement sear housing block from a .45 performed flawlessly at range. Wife's first time using my M&P and the result looks pretty satisfactory to me.
