My 625 4" had the same symptoms. The chambers were too tight. I bought a .45 ACP revolver reamer from Brownells and reamed them my self. That cured the problem as I can can now fire it all day and no cylinder hangup. It is one of my favorite revolvers.

I would ask several others to shoot the gun. Each of us grip with different amounts of strength; our hand sizes vary in size; we have different body mass. All of these can affect the point of impact using the same gun with the same ammunition.

It is expensive but mooning and demooning are fast. The two components are light in weight.
Make sure the primer tube is clean. I push ear swabs dunked in alcohol through the primer tube every time I switch between large and small primer loading. It can get surprisingly dirty.

FWIW, I've found my 625-8 does best with lighter (less than 225-230 grain) bullets. Of those I've tried .452 inch 200gr RNFP flat-based bullets with an BHN of 18 seem to give the best accuracy. The barrel rifling doesn't look to deep so my guess is the hard alloy helps get the bullets spinning...
David Sinko answered that one. If you understand Jerry's DVD and you have a proper set of screwdriver tips you'll be fine. I used a small needle nose plier to pull the pin that retains the firing pin. Just don't let the factory firing pin get away from you as it is lightly spring loaded.
