Update on fix. Maybe this will help others with the problem. I removed the large elevation adjustment screw on the top. The top of the sight folded back, revealing two tiny screw holes. Tightening these screws did the trick.
So what holds the rear sight on the Model 41? Mine is loose and easily slides off. The screw on the top is for elevation adjustment. I don't see a pin anywhere. Any help is appreciated.
So I think the problem is this pin in the front part of the barrel that engages and holds the ejector rod. Dead giveaway is when it fell out with the spring. I put them back, probably not the right way, and things seem to be working quite a bit better but still not perfectly. I suspect this...

Thanks Richard. I just tried to clean any debris but the problem persists. The extractor rod rotates freely in sync with the cylinder. If I hold the cylinder the rod does not move.
I am pretty new to this so am not entirely sure. If I hold the cylinder and try to turn the tip of the rod, nothing moves. So I assume the rod is not unscrewed.

I recently acquired a 686+ 2.5 inch and could not be happier. I am surprised at how well it balances and shoots, and it is fun to take to the range. I can't compare it with the seemingly more popular 3 inch as our wise leaders in California don't allow us the choice.
The 686+ 4" is hard to come by in California. Local dealer called and said he just got one in. How exciting! So I run down to look at it and find a very noticeable canted barrel. I can't see how anyone at S&W who looked at this would have let it out.
