Default 29-5 timing problem?

After about 1000 shots I have experienced 2 failures to fire. Both were using factory (Magtech) 44mag ammo.
The gun is a 29-5.

In both occasions the gun was loaded with 5 cartridges. In both occasions at some point the cylinder rotated two steps when shooting double-action. It "jumps" over a cartridge and locks at the next chamber.

Firing pin marks are smack middle. When slowly pulling the hammer back, the cylinder locks nicely and correctly couple of millimeters before the hammer is all the way back.

The problem never occurs when dry firing double action, or when using 44spl ammo.

How to fix? Is there a website or FAQ about timing problems? Or is this something else? The occurrence is so rare that this is kinda hard to debug...

Problems like that occurred in some of the earlier M29s when shooting heavy loads, but I thought those had been pretty well solved by the -5s. I have a M29-2/overstrike 3 that used to exhibit strange performance with hot loads, but I managed to make a home-made fix that worked OK. Still, I shoot mainly .44 Special-level loads, and haven't had a problem since. If you search around on this site, there's been a number of postings about problems with earlier 29s.

When was the last time it was tore apart and cleaned? Sounds like crud or a weak spring. The cylinder lock is slow on return for some reason, most common is the two reasons above.
