Failure to extract-645

I’m having issues with my 645. Have 3 mags. All are S&W mags w red followers. 2 came w gun when I bought it used. One has spring release at bottom other 2 do not. The problem: with any mag and regardless of loaded round count in mag, the last round in mag that is fired does not extract. It remains in chamber. It looks like the follower is too high and keeps the brass from being extracted. Remove the mag and the brass can be picked out or it simple falls out. Issue generally only happens on last round but it did happen a few times in the middle of string. Fired round doesn’t extract and next round tries to chamber. As best I can tell It looks like the extractor is ok. Any thoughts or recommendations?

As always, consider the condition and fitness of the recoil spring, the magazine springs, and whether or not the gun (especially the chamber) and the magazines are clean and undamaged, and the gun is properly lubricated. If you feel that the extractor appears OK appearance wise, keep in mind that a damaged or weak extractor spring can also cause issues, since it tensions the extractor itself. Recoil springs are cheap, I would start there.

Ammunition can also be a factor. Usually, this variable can be easily eliminated by trying a different brand or load.

Could be underpowered ammo and the slide is not fully cycling.
Check the ejector lever, make sure it is not broken.
How long has it been since you replaced the recoil spring?
Magazine springs could be getting weak.
The extractor channel may be full of gummy oil and powder reside.
The extractor spring may be weak.
 
Check all the mechanical things noted above, possibly check with Numrich/Gun Parts Corp to pick up new followers (plastic followers can wear).
 
Take a close look at the extractor. In a 1911, last round fte is an indicator of insufficient extractor tension.
 
Of the 6 strings shot, 4 of 6 FTE occurred on last round regardless of loaded round count. 2x occurred mid string. I am curious how to improve or modify the extractor spring tension as suggested. Going to do a thorough takedown and clean gun and mags. Also get a new recoil spring and go from there. Thx for feedback.
 
The only way to change extractor tension on these pistols is to change the extractor spring. It is a small, coil spring located near the rear of the extractor.
