It looks like there is a burr on the face of the bolt. What does the face of the catch where the bolt stops look like? The bolt catch always wiggles a bit. Does it move freely with the upper and lower separated?

Never ever use thread lock on a muzzle device. You will hate life. Always use a crush washer. They are made to keep the flash hider tight, yet allow removal when needed. They are good for only one use, but at only around $1 it's no biggie.

The extractor and ejector are completely different parts that have different problems. Failure to extract the spent case is an extractor problem (and possibly a chamber flaw) Failure to eject is an ejector problem. Any 1/2-28 device for 5/8 long threads will work. Some pistol devices are made for .45 long threads.
