Default Fail to Fire (FTF) and other related terms

I will never profess to be a firearms expert, with that in mind I've seen many terms for a firearm that fails to discharge it's round as intended. I would like to get a better understanding of what the terms actually mean. Please feel free to both define the terms or add new ones.

FTF (Fail to Fire) - I think this is a "catch all" myself
FTF (2) - (Fail to feed ?) -

I seem to recall several others.

The last time out my M&P 15 would not chamber the followup round after firing the previous one. It happened about 10 times out of a box of 50 rds. I was using the same Fiocchi (sp?) ammo I've used since I bought the gun but maybe this box was bad for some reason. If it does it again next session I'll send it back to S&W. Yes, I clean my firearms after every range session, it was clean and it's been cleaned since.
