Default M41 Broken Breech Face

My 41 has been acting poorly - misfeeding rounds into the top of the chamber. Took the breech block out today to clean up around the extractor and a wedge-shaped piece of the breech face fell out. Broken clean off.
On its way back to Springfield (cust. service said to send it to Springfield rather than Maine.)
Has anyone else ever had this problem? I've never heard of it. Gun is faily new - serial # UAT 90xx.

MJF - a first for me. Have seen soft slide bodies and rail breaks, but that is a first - Assume you sent complete pistol and not just slide. Could be related bbl chamber area damage. PLEASE keep us posted on S&W report.

Got the gun back from Springfield today - 13 days since I shipped. They replaced the bolt. No charge.
Excellent service - I'm very pleased.

I have several model 41's and never seen, had, or even heard of that problem.

The model 41 is made to shoot thousand and thousand of rounds.

S&W has been using MIM parts lately and this may be one???? The older bolts were stronger than the slide and bbl.
