The only sight I have seen on a S&W semiauto that is sort of like that is a Wilson Commando sight installed on a PC 952-1. It has similar "Right" and "Raise" indicators next to the screws, but it lacks the index marks and seems to be clamp-mounted to a base rather than equipped with a trapezoidal foot that would be drifted into a slot on the slide.
The index lines are sort of Colt-like, but I suppose any manufacturer could have included them as a common-sense design element.
In my lack of experience I suppose these might be factory sights. I just haven't seen enough S&W semiautos to know some of the standard attachments.

I remember seeing those quite a few years ago at the gun shows, they are a sort of a cheaper version of a BoMar, they were made as a "drop in" replacement for factory fixed sight pistols.....I cannot remember the make unfortunately, but I did have one on a Colt Gov't model, they made them to fit a few different models....

Did some digging in my parts box, it looks like an MK sight, they were made in Italy, I believe they went out of business and then were made by LPA......

I did a bit of google searching on millet, Mk, and LPA. Nothing looks exactly alike but the LPA versions that I am seeing look to be about the closest thing. The one thing about LPA is that most of them look to be marked LPA on the right side. It's a slick little design regardless of who made it. Thanks so far for the replies, they've certainly steered in the right direction.
