So, any ideas why or what to do? Cylinder of 6 rounds fired successfully 100% of the time in single action but only ~ 2/3 of the time (~4 out of 6 rounds) in double action.

I imagine that the first thing to do is remove the sideplate and clean out any gunk. Except being in .22 and in 95% condition suggests that there shouldn't be much gunk.

Thoughts?

There is a very small difference in the total hammer fall betw SA and DA.
The DA being less than the SA drop distance.
It can mean the difference in what you are experiencing.

Anything from lube,,to the mainspring tension spring backed out a touch.
The main spring tension screw having been shortened a very small amt.
Main spring lightened by thinning ...

Sometimes it's just the ammo,,especially with the lower quality of some of the stuff presented to us these days OTC.
A batch with brass a little harder to dent and missfires occur.
Place then under the SA mode or in another firearm with a stronger firing mechanism and all seems well.

I suspected that the single action position of the hammer was further back just enough to strike the round with enough force to detonate the round, whereas in double action, it just wasn't enough force—some of the time. This is what I was shooting:

22 Super Extra - Long Rifle Standard Velocity Lead Solid Point | Aguila Ammunition

Aguila standard velocity .22 LR ammunition has worked fine in other vintage Smith & Wesson revolvers and pistols that I own.

You can compare the mainspring tension on the hammer of different guns by hooking a trigger pull gage on the face of the hammer, lowering it to touch the frame, then taking a reading as the hook/hammer just starts to move backward. That will tell you the hammer fall force on each gun. Then you can see any difference between the ones that work all the time, and the one that misfires. Without measuring, you are just guessing.

Check the main spring tension screw, the 22/32 I just picked up had a great double action, I checked and the screw was loose, it took almost 2 turns to get it seated correctly.
