30-06 I bolt misfire with safety

I have a S&W 30-06 I bolt that after I shot and ejected the cartridge and reloaded the rifle, it did not fire. I pushed the safety forward and it fired. This happened three times. The safety was always in the fire position. Any thoughts on what is going on.
