A little help with remembering a flawed rifle..

So, my memory isn't what it used to be.

There is a bolt action or straight pull rifle and if I remember right, if the bolt or something was assembled wrong, the rifle bolt could fail and come back into the face/eye of the unfortunate person who was shooting it.

Help me out and tell me which rifle that was? My internet searching has just provided a bunch of clickbait and polemics.

Later Ross rifles had a solution to that. They modified the bolt so it couldn't be inserted wrong.
 
The Ross bolt could be reinserted and fired in the unlocked position.
Its biggest failing was that it was too sensitive to the mud and muck of Flanders where the Canadians fought
