As large as the 5" M&P in length and height , appendix carry is not practical at all unless your one very large guy . A 4.25 will carry at 3:00 well and some can carry a 4" like a 2.0 compact or glock 19 size but how well that works 24-7 is your problem to figure out . I have carried my wifes 4.25 CORE with a RMO5g on it in a Comp-Tac Neutral . Today that would be there MTAC . For me a 4.5 or 5" anything is to long to carry comfortably and draw quickly when compared to a 3.5 to 4.25 pistol

I have a Yaqui style holster for when I want to carry that way. Mostly use shoulder holster.

I carry mine strong side (right hip) with a modified Cross Breed Holster to except my Fast Fire 3 red dot.
Very Comfortable even when sitting.

Never understood how anyone with a desk job can sit on a gun at all. I pocket carry my P365 for this very reason. I am a bigger guy though so maybe that's my problem. When I worked in gun sales I wore my XD9 Mod.2 Sub compact OWB and I spent at least half my day on my feet and I found that tolerable. But IWB? Almost never comfy with it.

respectfully disagree. carried a government model iwb (or owb, depending on situation)with nary a problem.
We're not talking about IWB, we're talking about AIWB. Like this:

I think it's dumb to point a loaded gun at your winkie, but some people think it's OK. Put a 5" gun in like that and you either stand or have to have wear your pants like this:

Lots of people carry a Glock 17 AIWB all day and it is only .5 of an inch shorter. If you have the right body shape and holster for it you can do it. In fact a lot of people use a G17 holster to carry a G19 because the extra length helps keep the butt from rolling out.

Personally I have and like the JM Custom Kydex AIWB 2.0. I have one for a 5 inch M&P and can carry it fine.

I appreciate the holster recommendations. I have a 5" Pro, as stated above as well as a Shield 9mm that conceals well but I worry about capacity. If I can find a holsterto carry the pro, even only in cooler weather and more clothes, great. My other thought is to sell them both and replace them with a Sig 320compact Rx. One gun for home and carry, with Romeo 1 red dot. Thoughts??

No personal experience with the Sig 320.

The Sig Romeo is a middle of the road optic with many weaknesses. I would not use one for serious heavy duty personal defense.

The RMR line is still the top of the picks. One other RDS manufacturer has been bright enough to duplicate the RMR footprint: Holosun. Cheaper by $250 if you can find one. New enough to still have some teething problems and as with all others, just not as robust as the RMR. But when you tire of it, or get enough flow, you'll buy an RMR to fit your custom milled slide.

I wouldn't base my gun decision on a Romeo.

Dropped from 2' onto hard surface:

HA HA HA,,,,, right! And as for the pointing issue, it's like the pilot's saying......"why jump out of a perfectly good airplane..?"

I love the idea of AIWB for concealment and fast access but, I can't get past having to draw the gun out while it's pointed at my most precious organ lol.... I think you're barking up the wrong tree with a 5" gun AIWB, pickup a 3.6" M2.0 compact or better yet a shield for that duty...

This barrel extension makes this 4.25" just short of 5". I carry it AIWB almost every day. Trigger pull of 5.25#. I don't hitch up my pants. I can sit down. I'm 5' 11", 185#, average size. Stiff nylon belt, JM Custom Kydex holster. Spare 21 round mag (Taran Tactical extension) on the offside at 10:30, fixed blade push dagger at 11:30. The right components make this very possible.

No, in over two years of AIWB carrying, I've never had a negligent discharge. And yes, I've jumped out of a perfectly good airplane, 574 times.

I holster slowly and carefully with my hips tilted forward a bit and I look the gun through a clear path to the holster—like everyone should do carrying in any position. I have a foam wedge on the body side bottom of the holster. It helps tip the barrel away from me and the grip inward. When holstering my barrel is not pointed at my precious parts. Once the gun is properly in the holster, it's not going off by itself. Outside of the holster I use consistent trigger finger discipline, as we all do for safety.
