Extractor issues

Was at a match today 7-24-22 and had a malfunction seemed to be a double feed. I went to release the magazine and the pistol discharge as the magazine was coming out and the bolt went forward. I didn't have my finger on the trigger because I can't push the magazine release without rotating the pistol I have arthritis and I have to completely move my hand so I can press the release. So I was DQ fine I understand that part. When I got home was impossible for me to move the bolt back more than a 1/4 inch so I removed the take down screw took off the upper assembly and removed the bolt. Upon inspection I noticed the extractor and spring where gone? Is this a common occurrence I'm looking at getting the tandmakross kit to replace this but still how does that happen when the bolt is still in the firearm and why did the gun discharge?

Sir it would help if we knew exactly what weapon we're talking about, a 41? a 52?
