Failure to load round....

Can someone post suggestions or a thread that might help me?

I have an M&P 9 Pro. I've put about 5,000 rounds through it and use factory magazines.

Initially, I had two problems.

One was, the spent round would not be extracted from the chamber and the second problem was I would also get a failure to feed a round during a magazine change.

I've sent it to S&W and they did some work on the barrel and installed a new extractor.

I have not yet have any failures to extract but yesterday, during an IDPA match, two of my 6 magazine changes did not load a fresh round. I had to rack the slide to get one in there.

Any advice?

Also, if I briskly slap a magazine in, the slide will charge forward without me having to touch the slide lock lever.

Is this normal?
Could this be contributing to my problem?

The feeding issue could be mag issue since the top round wasn't picked up by the slide. Maybe time for some Wolff mag springs? I've had several autos that would drop the slide when slapping in an new mag and have never had any issues, I'm sure someone else will have more info though.
